I, the undersigned, in exercise of my “freedom of choice” and other common law constitutional rights, do hereby give my consent to and appoint Damaris Drewry, Ph.D., as my Mentor, EFT Practitioner, Shamanic Practitioner, and Intuitive Coach. I understand that her method of working with clients is experimental and not all people experience the same results.
In doing so, I hereby guarantee the following conditions:
1. I request this assistance privately, and I am NOT requesting this service as a vehicle of public or government investigation nor utilization, either directly or indirectly.
2. I understand and accept the fact that Damaris Drewry is not licensed as a psychologist, that she DOES hold the Degree of Doctorate in Psychology, and that I am giving her my voluntary consent to proceed in assisting me under my common law constitutional rights.
3. If I make use of the assistance from Damaris Drewry, I will because of my own personal convictions, discretion, and sincere desire to do so.
4. I understand that there is a risk of emotional and/or physical distress which may arise after the session, and hereby waive, release and discharge any and all claims of damages for personal injury.
5. My payment at the end of the session indicates my satisfaction with the results in the session. Any funds that I give to Damaris Drewry are free will in nature.
6. I agree to release and hold harmless Damaris Drewry from any effects resulting from session(s) and at any time following the session(s) and it is further understood and agreed this waiver, release and assumption of risk is to be binding on my heirs and assigns.
